Venous insufficiency shows up in different ways, all of which may benefit from vein treatment. These may include external physical signs such as spider veins and varicose veins, although several other signs or symptoms may include the following:

Venous insufficiency occurs when the veins have trouble sending blood from the legs back to the heart. This increased pressure in the leg veins causes the veins to get bigger and more visible. Several vein treatment options are available to treat venous insufficiency before it evolves into something more serious such as lipodermatosclerosis or {stasis dermatitis} an inflammatory skin condition, due to leaky veins, that can cause recurring skin infections. This can also lead to venous ulcerations, which can be very dangerous.

A condition in which a blood clot forms in a vein, venous thromboembolic disease includes both deep vein thrombosis (DVT) and pulmonary embolism (PE). DVT typically occurs when a blood clot forms within a deep vein in the body (typically in the legs). PE is the result of a piece of DVT breaking loose and traveling into the lungs, which can be fatal. Risk factors for VTE include both venous insufficiency and iliac vein compression syndrome.

A common cause of deep vein thrombosis (DVT), iliac venous compression syndrome occurs when the right iliac artery compresses the left common iliac vein in the pelvic region, resulting in a reduction in the venous outflow. These blockages in the pelvic veins cause blood to back up and pool in the legs which can lead to the various signs and symptoms of venous disease, as well as DVT, VTE, and pelvic congestion syndrome (PCS).

In the treatment of varicose veins, microphlebectomy (removal of veins through very small incisions) is the most common “surgical” procedure performed. Microphlebectomy is an outpatient procedure which takes about 30 minutes to perform, and is often performed in conjunction with endovenous ablation under local anesthesia. Some varicose veins will resolve or shrink after the underlying problem vein has been sealed with endovenous closure. However, larger veins may require removal at a later date. Before endovenous closure was introduced in 1999, the general perception of varicose vein removal procedures was that they were only a short-term fix and that other veins would start bulging within just a few months. The incisions used in the past were also larger and less cosmetically appealing than the tiny incisions (micro) currently employed. With the modern techniques available today, including accurate ultrasound mapping of leaking veins, endovenous closure of the underlying leaking veins, and skillful surgical removal of large bulging varicosities through very small incisions, one can be much more certain of a favorable and lasting result from varicose vein or venous insufficiency treatments.

Intravascular Ultrasound (IVUS)


Intravascular ultrasound (IVUS) is a medical imaging methodology using a specially designed catheter with a miniaturized ultrasound probe attached to the patient end of the catheter with the other end attached to computerized ultrasound equipment. The end with the probe is inserted through an IV in the leg and directed into the pelvic veins of interest. IVUS allows for examination of the veins from within to check for obstructions. Precise localization and accurate measurement of the severity of the vein narrowing is possible with IVUS.  Although stent technology and IVUS have been used for years in other vascular beds, the use of them together to investigate the pelvic veins is new and exciting in that many desperate vein patients as well as patients with undiagnosed pelvic and buttock pain previously evaluated by their OB/GYN or Pain Management physicians now have the potential of being helped. 

Intravascular ultrasound as it applies to the venous system is focused on the identification of iliac vein compression syndrome (IVCS). One form of this is referred to as May-Thurner syndrome wherein the left common iliac vein is compressed between the crossing right common iliac artery and the 5th Lumbar vertebral body or the sacrum.  The chronic, pulsatile compression of the proximal left common iliac vein by the right common iliac artery against the spine leads to peri-venous fibrosis or scarring.  Blood flow from the legs has to find alternative routes of return through the pelvis to the heart, and these pelvic collateral veins result in the condition of pelvic venous congestion. Left sided lesions account for 75% of these cases while right sided iliac vein compression lesions account for the remaining 25% of cases seen all of which may benefit from IVUS evaluation and treatment.  Thus, it is imperative to examine both iliac venous systems routinely when investigating for iliac vein compression.  For example, a left sided iliac vein compression may present with right sided leg signs and symptoms and the left sided symptoms may (less frequently) be caused by right sided iliac vein compression lesions.

The symptoms of vein disease can often be misdiagnosed without further testing. Untreated chronic vein disease can lead to more advanced signs and complications such as thickened skin, infections, bleeding, and ulcerations.  Chronic Venous Insufficiency (CVI) is another name for venous disease and occurs when the valves in your leg veins fail to close tightly and allow blood to pool in your legs. Although usually not life threatening, CVI can result in significant medical complications. Deep Vein Thrombosis (DVT) is the most serious medical condition that can result from untreated vein disease. It occurs when a blood clot forms in a varicose vein and extends to one or more of the deep veins inside the muscles of the leg or thigh. DVT can lead to life-threatening pulmonary embolism (blood clots in the lungs) and must be treated promptly to prevent the potentially fatal complication of pulmonary embolism.
