PDA can be challenging to evaluate, and especially with young children, the line between what is developmentally appropriate and what is potentially indicative of PDA can be a fuzzy one. To evaluate and diagnose a child with PDA, Dr. Myszak prefers for a child to be age 5 or older, though sometimes exceptions can be made for children with a history of participation in a structured school setting.
Children ages 6 and under should plan for in-person testing. Other types of assessments, such as pre-adoption evaluations, often require in-person testing. Other forms of testing can often work just as well over telehealth.
Upon your request, I provide documentation (a super-bill) which details the services so you can submit it to your insurance company for out of network reimbursement. Depending on your insurance benefits, your deductible, and your out of network coverage, you may be able to get most of this cost reimbursed to you, but it is your responsibility to call your insurance company to ask about your benefits. (Additionally, due to many insurance plan’s high deductibles, the out-of-pocket cost may differ very little from what you would pay for an insurance-covered evaluation.) The testing billing codes to call and ask your insurance company about are 90791, 93132, 96136, and 96137 (it would be multiple units of the last 3 numbers.) For therapy, the code is 90834. Some insurance companies require a pre-authorization form to be completed before testing to justify the need for a psychological evaluation. I am happy to complete this at my hourly rate for submission if you let me know what is required.
Yes, Dr. Myszak is able to complete documentation for testing pre-authorizations. If you send her the form that needs to be completed, she can do this paperwork after an initial diagnostic evaluation at her hourly rate. If this is important to your family, it may make sense to schedule the diagnostic evaluation and then hold off on scheduling additional appointments for several weeks, since it can often take the insurance company some time to process paperwork.
You are in the right place! If you are scheduled for a C & P exam, at your appointment I will ask you about your background and history and ask you about any mental health difficulties that you are experiencing. The appointment typically lasts 1-1.5 hours and you are welcome to bring along a family member or close friend if you choose to. (If you are a Veteran coming in for an evaluation, please do not schedule a phone consultation beforehand—we will get through everything we need while you are here.)
This depends. My goal is to help you get better as soon as I can, but everyone is different. Sometimes people benefit greatly from just one consultation appointment, and other people might need several months or more. Generally, people start with weekly therapy, and then gradually space appointments out as they start feeling better. Sometimes people might feel better and then come back for a “booster session” or two later.

A psychological evaluation involves the collection of information to determine whether there is an underlying reason for an individual’s difficulties (i.e., learning difficulties, autism, ADHD, etc.) Depending on the reason for testing, a variety of testing materials are used—-structured interviews, standardized tests of development and cognitive functioning, daily living skills, and other abilities. Testing typically involves several appointments of testing/data collection and all results are written up in a report with recommendations.
